Simply put, I didnt break any blocks that I know of. I did place a torch to open a trade booth at one point so people could actually use a trade booth.

What blocks did I supposedly break? and Where was my first offense warning?

You're now unbanned. I talked with italia about this, and he says you broke a glass block. I can neither attest to this, or deny it as I wasn't online/haven't seen the logs.

Italia had banned a large number of people that day for griefing spawn, and I think you just sorta got caught up in some collateral damage.
